چکیده
of Purpose: A comparison of long term effect on IOP and complications trabeculectomies performed with the adjunct of either postoperative 5 fluorouracil(5FU) or peroperative mitomycin C(MMC) in 100 cases of high risk glaucoma. &&&We prospectwly studied 52 eyes of46 patients treated with MMC (Follow up 15i8.7111) and compared it with a retrospective scrie of48 eyes 01 35 patients treated with 5FU (Follow up 14,3li 11,25m). &w&84% of MMC patients but 46% of 5FU group had a normalized 101’ without treatment, and 23% of 5FU vs 5,7 % of MMC patients required systemic glaucoma therapy. Early adverse effects were more’frequently observed with 5FU (cornea 39%, conjonctiva 17% or hypotony 29%) than with MMC (13,46%, 3,85%,13,46% rcspectivly). At last examination, bleb failure requiring additional surgery was more frequent in 5FU group (10%) than MMC (3%), and was mostly associated to anterior chamber factors (i.e. vitreous or lens rclatcd obstruction), rnthcr than cpisclcrnl cclluln~ li~c~owl.o~~g km hypotony was obscrvcd in il l 17.3 % ol the MMC cycs. in this later group 5 of 9 patients had more than 2 lines of best corrected visual acuity reduction. Conclusion: l’hesc results suggest that MMC is ot Icut as cl’licicnt 10 reduce lOP.The use of MMC may be associated with a higher rate of long term hypotony.

Additive effect of dorzolamide on aqueous humor flow in patients receiving long-term treatment with timolol.
OBJECTIVE To determine the additive effect on aqueous humor flow of short-term dorzolamide treatment in patients with glaucoma receiving long-term treatment with timolol. SUBJECTS AND METHODS Thirty-nine patients with glaucoma, 19 at Mayo Clinic, Rochester, Minn, and 20 at the University of Uppsala, Uppsala, Sweden, who had been receiving timolol treatment in both eyes for at least 1 year wer...
متن کاملOcular pharmacokinetic/pharmacodynamic modeling for multiple anti-glaucoma drugs.
We have constructed a new ocular pharmacokinetic pharmacodynamic (PK/PD) model for anti-glaucoma drugs to describe ocular hypotensive effects on intraocular pressure (IOP) after instillation of a combination of an alpha(1)-adrenergic antagonist, bunazosin, and a beta-adrenergic antagonist, timolol, into rabbits. This model was constructed by the combination of two ocular PK/PD models for bunazo...

OBJECTIVE To evaluate the interaction of intraocular pressure(IOP)–lowering medications with physiologic day and night changes in aqueous humor dynamics in participants with ocular hypertension. METHODS Thirty participants were enrolled in thisdouble-masked, randomized, crossover study. Each participant underwent aqueous humor dynamics measurements at baseline and at 2 weeks of dosing in rand...
متن کاملOcular pharmacokinetic/pharmacodynamic modeling for timolol in rabbits using a telemetry system.
We have established an ocular pharmacokinetic/pharmacodynamic (PK/PD) model for a beta-adrenergic antagonist, timolol, after instillation into rabbits. Timolol concentrations were determined by HPLC in the tear fluid, aqueous humor, cornea, and iris-ciliary body after instillation or ocular injection into the anterior chamber of the eye in rabbits. In addition, intraocular pressure (IOP) measur...
متن کاملExpression of Cytochrome P450-enzymes and Metabolism of Timolol in Human Ocular Tissue
Timolol has been used for decades to lower increased intraocular pressure (IOP). However, only limited information is available on the metabolism of the drug, especially that in the human eye. The aim of this work was to investigate the expression of drug-metabolizing cytochrome P450 (CYP) enzymes in the ocular tissues, the absorption of timolol into the aqueous humor from two commercially avai...
